Mission

Health People (HP) focuses on tackling pressing health and social challenges in underserved Bronx communities. HP addresses high rates of chronic disease in the Bronx. Major health issues targeted include the Bronx’s exceptionally high rates of chronic disease including diabetes, heart disease, hypertension, lung disease, arthritis, childhood asthma, and chronic pain. Major associated SDOH addressed are housing, food insecurity, poverty, youth and adult unemployment, and low educational attainment. These conditions occur far more frequently in The Bronx than in other NYC boroughs. By empowering community members as peer educators and advocates, Health People works to break cycles of poor health, limited opportunity, and health disparities that are rooted in social and economic inequities.
